Radecki denies creating the documents, but both his account and patients connect to them. Lena is not the first person whose death seems to reach the system before reality. An officially dead patient appears alive, another woman vanished after a suspicious referral and a hidden facility operates outside normal hospital procedures.
Behind the events is a data-migration project capable of changing more than an individual record. The question expands from whether someone intends to kill Lena to what can happen when enough systems already treat a living person as dead.
